Local family-owned Home Health and Hospice Care of Nursing Placement and Health Concepts Ltd., headquartered in Pawtucket, announces the purchase of Crystal Lake Rehabilitation and Care Center, a 71-bed nursing home located at 999 South Main Street in Pascoag, Rhode Island.

The amount of the transaction was not disclosed.

The acquisition brings the number of skilled nursing homes that Nursing Placement and Health Concepts owns and operates throughout the state to seven, including Village House Nursing & Rehabilitation Center (Newport), Elmwood Nursing & Rehabilitation Center (Providence), Bayberry Commons (Pascoag), West Shore Health Center (Warwick), South Kingstown Nursing & Rehabilitation Center (West Kingston), and Eastgate Nursing & Rehabilitation Center (East Providence).

“We are excited to ensure Crystal Lake residents continue to receive the same compassionate care in their nursing home, and plan no changes to staffing or services,”states Health Concepts Chief Operating Officer Kelly Arnold. “Our quality commitment to those in our care and their family members is to provide stability from a legacy team familiar with their needs, routines and preferences so the transition was seamless.”

Crystal Lake Rehabilitation and Care Center was founded in the early 1970s as the Jolly Rest Home. Several decades later, it was renamed Pine Grove Health Center and in 2019 became Crystal Lake serving residents of Pascoag and neighboring towns including Foster, Glocester, Smithfield, and Nasonville.

Today under Nursing Placement/Health Concepts’ ownership, the nursing home will retain Crystal Lake’s 70 employees plus per diem staff, and continue to offer 24–hour care in long term care; short term rehabilitation; physical, occupational, speech and respiratory therapy; pain and wound management; and palliative and hospice care services. Rooms are semi-private and private, and Medicare, Medicaid, Veteran benefits, and most insurances are accepted.
